Luffy first activates Gear Second during the Davy Back Fight to defeat Foxy. He then uses it again during his battle with Rob Lucci, and in most subsequent battles. Luffy can use Gear Third at will but only for one minute before he becomes exhausted and so cannot use it against opponents stronger than him such as Crocodile or Enel. When using this mode while fighting an opponent weaker that himself, Luffy displays some degree of control over when he loses consciousness due to overexertion.
Gear Second: This is the second form of combat gear developed by Monkey D. Luffy; activated with a roar after imbuing Haki into his body – though not all users are capable of activating their gears in this way. Luffy also has been shown using gear second on some occasions without going into gear third as soon afterwards, such as during his first fight with Crocodile (though it should be noted that Luffy barely won). He can also use this tactic while fighting underwater.
In previous occurrences where Luffy had used two gears consecutively without entering the next stage immediately afterward, he either dropped unconscious because there was not enough time or will to time his usage of the gears correctly, or he deliberately chose to enter a particular gear in order to use it for the duration.
In One Piece Manga Chapter 282 and Episode 204, Luffy revealed that he can use Gear Second at will without having to go into Gear Third afterwards (though not simultaneously). This is due to him having more than enough willpower.
This means that now as long as there’s still some power left after using Gear Second then Luffy may activate it again if necessary. There are also instances where this does not work out well because unlike with Crocodile when he used two Gears consecutively without entering any other one immediately afterward so they were successful but against Rob Lucci when Luffys stamina didn’t last long enough.

In One Piece, Luffy has only entered Gear Second twice in the manga and three times in anime. The first time was on Sabo after being told to “wake up”. The second time is when fighting Kizaru commenting that his punch can’t hurt him; which as proven wrong. In terms of who triggers his gear seconds last or first we do not have a definite answer at all because everytime luffy uses gear second someone else gets injured and these people may deserve it depending on your point-of-view but most likely don’t if they are innocent bystanders.
When does Luffy use Gear Second? When angry enough (usually by an opponent saying something) or when he is hurt (usually when the opponent causes that injury).
He enters Gear Second, his haki increases greatly and all of his physical abilities increase. Luffy’s hair also becomes blond from the speed up in metabolism. He can do more powerful melee attacks with a wider range at an incredible speed.
A punch from gear second will usually cause major damage to most opponents including Kizaru who underestimated it because he had never seen such power before but was not strong enough to stop him; though no one has been able to survive a full powered attack and get back up yet.
